Nurses are faced with ethical dilemmas on a daily basis, each situation being unique and requiring the nurse to set aside their own values and beliefs in order to properly care for their patients. Situations requiring nurses to make an ethical decision are diverse and dynamic; the values set out by the College of Nurses of Ontario code of ethics remains the same. Therefore, all decision based on these vales regardless of the setting and circumstances ensure consistent solutions. The scenario involves a woman who was admitted to the NICU due to complications during her sixth month of pregnancy. She told the nurse that she was not enjoying her pregnancy but believed that she will learn to love her baby once it is born. She went on to state that she had not discussed her feelings with her husband and family and asked the nurse to keep the conversation private. The nurse had been in the nursing profession for fifteen years and working on the NICU Unit for five years. She was of Caucasian background and a single mother of two young children. She first met the patient when she was admitted with complication during the sixth month of her pregnancy. The nurse had extensive training and experience working in the paediatric unit of the hospital where she was employed. Caring for children is her passion since she lost her first child due to birth complications and raised her two children alone while putting herself through university. In the 25th week of her pregnancy, the mother was advised by the nurse to remain on bed rest to avoid further complications and potentially hurting her unborn baby. The mother continued to work from the hospital placing additional stress on the baby despite the nurse’s appeal that such stress can cause the baby harm. Attempts to stop premature delivery were made but failed, the mother asked the medical team not to take any extraordinary measure to save the baby. The topic of ethics is prevalent in health care and addresses a broad range of topics in nursing. In almost every interaction with a patient there could be a situation that may bring up the question of ethics. Fortunately, there is the realization that placing the nurse in the care of a patient, may put the medical personnel in an environment where the ethics are questioned. There are whole departments dedicated to advising nurses in these situations. There are also ethic committees to help guide you when you find yourself in a situation that may question your practice or circumstances.

Twenty-one years ago, my parents received the devastating news that my lungs were not developing properly and as a result my mom had an induced labor. I was born as a premature baby, weighing only 3 lbs. and was placed in the care of neonatal nurses in the Neonatal Intensive Care Unit (NICU) for 18 days. While my mom came to visit me, she would notice how well the nurses would care for me. Though their jobs were tough, the nurses would often carry me while I was crying and would often talk me to sleep, sometimes singing lullabies.
Nurses rely on personal knowledge and their professional skills to provide ethical care (Creasia & Friberg, 2011). In everyday practice, nurses must balance the needs of their patients against those of the organization, society and themselves. They strive to deliver the highest level of care for patients, but adjusting for limited organizational and personal resources often requires difficult decisions. This paper explores the following scenario suggested by Maville and Huerta: “You are a nurse providing home care to a mother, and you suspect child abuse after observing the mother’s reaction to her child” (as cited by Arizona State University, 2014). When faced with a moral dilemma, a competent nurse incorporates ethical, bioethical and legal considerations. In the proposed story, incorporating the nursing ethics of advocacy, beneficence, nonmaleficence and collaboration will guide the nurse towards an appropriate and legal course of action.
Deontology is an ethical theory concerned with duties and rights. The founder of deontological ethics was a German philosopher named Immanuel Kant. Kant’s deontological perspective implies people are sensitive to moral duties that require or prohibit certain behaviors, irrespective of the consequences (Tanner, Medin, & Iliev, 2008). The main focus of deontology is duty: deontology is derived from the Greek word deon, meaning duty. A duty is morally mandated action, for instance, the duty never to lie and always to keep your word. Based on Kant, even when individuals do not want to act on duty they are ethically obligated to do so (Rich, 2008).
The case of Baby Boy Doe is an ethical dilemma because it’s an occurrence where “decision makers are drawn in two directions by competing course of acting that are based of differing moral frameworks, varying or inconsistent elements of the organizations philosophy, conflicting duties or moral principles, or an ill-defined sense of right and wrong.” (Darr, K. 2011) There were many differing viewpoints as well as moral and ethical choices on this case, the parents, nurses, physicians and hospital as an organization. After declaring Jahi McMath brain dead, the doctors at the Children’s Hospital ordered her to be taken off mechanical ventilation. Her mother, Latasha Winkfield, refused to accept that her daughter was dead and incapable of recovering (du Toit & Miller, 2016). Winkfield believed her daughter was still alive because McMath was breathing and her heart was still beating (du Toit & Miller, 2016). McMath’s family disagreed with the doctors’ report and filed a lawsuit to prevent life support from being discontinued (Paola, 2014).

Accountability and Responsibility are two of the most important skills that a clinical nurse can display. These skills are taught beginning in nursing school, when learning about the ethical framework behind the nursing profession. Nursing is a skillful combination of science and art that places patient preferences above everything else. Not everyone has the unique ability to be caring and medically savvy. The American Nurses Association defines accountability as the ability to be answerable to oneself and others for one’s own actions (Battie & Steelman, 2015). Accountability plays a huge role in patient care and the process of learning the skills of nursing. Student nurses learn that nurses should be accountable for all actions, delineation of tasks to supportive staff, as well as nursing interventions. Responsibility is also intertwined with this idea of being accountable for one’s own actions. Responsibility refers to the specific accountability or liability associated with the performance of duties of a particular role. Nurses can either accept or reject specific role demands based upon their education, knowledge, competence, and extent of experience (Code of Ethics, 2015). This paper will discuss accountability and responsibility of nurses delving into assessing one’s own competence, techniques of learning responsibility, and the future of nursing accountability.
This hypothetical situation illustrates a common problem faced by nurses, as the roles we serve can be multifaceted and demanding. It is often tempting to cut corners, especially if we feel there will be no actual harm done. This, however, does not justify professional misconduct nor allow us to expand our practice beyond our designated scope. There are five principles to ethical nursing. The first principle, nonmaleficence, or do no harm, it is directly tied to a nurse's duty to protect the patient's safety. This principle dictates that we do not cause injury to our patients. A way that harm can occur to patients is through communication failures. These failures can be intentional or as a result of electronic or human error. Ethical issues in nursing will always be an ongoing learning process. Nurses are taught in nursing school what should be done and how. Scenarios are given on tests with one right answer. However, there are situations that nurses may encounter that may have multiple answers and it is hard to choose one. “Ethical directives are not always clearly evident and people sometimes disagree about what is right and wrong” (Butts & Rich, 2016). When an ethical decision is made by a nurse, there must be a logical justification and not just emotions.
Ethical problem solving and decision making is an arduous process for any nurse or health care worker. As Marquis and Huston, (2015) point out, that ethical decision making takes thinking skills and risk taking abilities to address and solve ethical problems. An ethical dilemma occurred during my charge in the NICU many years ago. A twenty-two week baby was born breathing and crying, it was decided by the team of neonatologists and the parents that no heroic efforts would be made to keep this baby alive. At the time the neonatal resuscitation guidelines for saving a baby were twenty-four weeks. Meanwhile, the baby came to the unit and placed in an isolette while the parents visited and held the baby. The case of Christi presented presents ethical problems people in the healthcare field face every day. In this case we are shown a women who is 22 weeks pregnant and suffering from preeclampsia (Panicola 119). Christi’s doctor recommends immediate delivery, despite the fetal consequences to the fetus. After considering her options, Christi ultimately decided to induce labor, because the potential risks to her health were too great. Cases like this offer ethical problems for both the client and their caretakers. Ethical issues presented in cases likes these effect both client and doctor/nurse.
Nurses are face with numerous ethical dilemmas each day and if theses ethical situation are not handled in a professional and ethical manner there can be severe consequences for both the nurse and the patient. When nurses are face with theses ethical dilemmas, they have a decision to make. Therefore, what does the nurse do when decision-making involves ethical dilemmas? So, has a nursing student, I have chosen to put myself in the role of a health care provider such as a nurse. It was my first day of clinical rotation and the client that I was assigned to, was due for a bath. I was not comfortable giving the patient a bath a lone, so I ask for assistance from one of my colleagues. The client was a male who was shy and soft spoken.
