I don't even know where to begin, but this is completely unacceptable. My truck TR137 has been down since the end of my shift on Thursday, June 23rd, on my post trip inspection I noted an oil leak and steer axle shock had come unbolted on the bottom, both on the passenger side, both of these items I noted on my VCR. I arrived at the yard on Friday, June 24th at my regular time. On my pre-trip inspection, I noticed that the shock was fixed and that the side of the engine with the leak had been washed, but on the VCR it was only noted that the shock was fixed. I questioned the mechanic on duty, Bill, about the oil leaked, he said they could not find it, but go ahead and run it, we'll see what happens. Well, I only made it two miles

TR124 has an oil leak, the air compressor is covered in oil. The hydraulic connections are leaking on the rear of the truck, so much that there was a puddle under it. See pictures below. Other DOT violations included no current fuel tax permit in permit book (although correct stickers are on the truck), tractor ABS light is on the dash, hydraulic hoses are secured to the truck with bungees, passenger door does not close properly.
Another issue that prevented the truck from being ready for operation was the locking bar for the fifth wheel plate, that is needed to run our dump trailers was missing. This brings us to the interior of the cab, and this unto itself is unacceptable! This is the amount of garbage I pulled out of the cab. In case you are wondering, that blue thing is a propane canister, that was just rolling around under the garbage along with an unsecured fire extinguisher and yes a 20lb dumbbell. I do not believe I need to tell you that the company's standing with DOT is not great. Saying this truck is ready for safe operation is completely unacceptable. Furthermore, my life, my safety, and my family's financial well-being, as well as every other driver, depends on proper maintenance of the equipment. On January 10, 2017, I, Officer LESSANE, with the city of Beaufort Police Department, responded to 1030 Ribaut Road #6, in the city limits of Beaufort, to conduct a follow up on a previously reported domestic (17B01907). Upon arrival, I made contact with the victim SOFIA CHAVEZ and confirmed the offender VICTOR MANUEL CRUZ-ANAYA information. While at the victim residence, I took updated photos of her injuries. She also provided me with a photo of the offender. After clearing from the victim residence, I relocated to the offender job in an attempt to make contact with him. Negative contact was made, no further actions were taken.
On 12/8/16 at approximately 11:30 AM, An Outbound Desk Clerk notified Security Account manager Enmanuel Cabrera that a yard jokey found a trailer with damage. Upon being notified, A/M Cabrera instructed Security Officer Larry Mayer to take pictures of damage and to gather the information needed. Upon arriving at the location where the incident occurred, S/O Mayer was met by RJ davis Yard Jockey Mark Billus JR. Yard Jockey Mark Billus JR stated that, while pulling XPO trailer W17150 out of dock door 6, he notice it was too close to dock door 7 for the TDR crew to properly seal the trailer. We originally did not recognise that the child had Foetal Alcohol Syndrome. The child was a slow developer and one who would have huge tantrums. We didn’t think anything of it but just thought some children develop skills slower than others and tantrum wise we put it down to, some children are just difficult but would eventually grow out of it. We only started suspecting something was different when the child started going to pre-school. If I had not had to go to work; then the child would have not attended pre-school but gone into school, when they would reach school age.

This officer responded to Priced Rite Tree Service in reference to a stolen vehicle. Upon arrival, this officer contacted Middleborn, who advised they just noticed their 1988 Chevrolet black pickup was missing. Middleborm advised the vehicle was stolen sometime within the last 3-4 days. Middleborn stated an employee was supposed to take the vehicle in for repairs. Middleborn stated he called the company and learned the vehicle was never dropped off. Middleborn stated he spoke with his employee and learned the employee thought the vehicle was already in for repairs and did not check with anyone.

I spoke with Ryan, who stated he was eastbound at about 40 mph when he stated he could not stop in time & struck the trailer. Ryan stated he was wearing his seatbelt & was alone in the car, a 2015 Nissan Altima bearing MN registration of ADT800. Ryan’s car had severe front-end damage & appeared to be stuck on the left frame rail of the trailer.
I exited my patrol vehicle and made contact with the driver of the vehicle Erica Manning 24 year old female, and the front seat passenger Brent Manning 30 year old male. I identified myself and asked Erica for her drivers license and proof of motor vehicle insurance. Erica removed her license from her billfold and handed it to me. I asked again for her motor vehicle insurance. Erica began looking around inside the cab of the vehicle. Erica began looking in the center console of the vehicle but was unable to locate the insurance information. Erica said she wasn't sure where the insurance is. Erica asked if I would accept the insurance in a digital form on her phone, but had difficulty finding words she wanted to use. Erica began using her phone trying to find her insurance information. Brent began looking into the glove box of the vehicle. While I was speaking with Erica and Brent I noticed they were both talking slowly with mumbled and slurred speech. Quickload Carriers is owned by Tomas S. Moreno. Mr. Moreno and Mr. Epps, Operations/Safety Manager, run the day-to-day operations and are both responsible for compliance with the FMCSRs. This carrier owns 1 tractor, leases 7 tractors (owner/operators) and leases 8 trailers from AMC Truck & Trailer located in Laredo, TX. Mr. Moreno goes to Laredo, TX approximately once every two weeks to check on the vehicles but has failed to obtain documentation from drivers or AMC showing when repairs were made and when preventive maintenance was conducted. Carrier has monthly vehicle maintenance logs but many of the logs are blank. Carrier has some repair receipts but they have no system in place to ensure that required repairs are made and that preventive
