My patient is n general an independent one in all aspects of her life and she would like to be in control in everything regarding her life. She is leaving in her nice apartment by herself but in the same time she enjoy being in company of other people, especially people on her age or closer. Therefore, I consider that the best community for her to attend will be a day care community, where she can go almost every day and she can spend some quality time with other older adults. I chose a day care community because I know she needs company to avoid episodes of depression that she has occasionally, accordingly with what she told me at one of my visits. Since post-hospitalization follow-up with primary care providers has a great impact on theses Veterans’ health outcome by promoting recovery and preventing readmissions (Martinez, 2014). The Patient Aligned Care Teams track Veterans’ admission and discharge in VA facilities through the VA’s electronic medical record to ensure timely post-hospitalization with Veterans’ primary care providers. There are no data about post-hospitalization follow up among Veterans who is admitted into Non-VA facilities.

The first few weeks of internship, at Ramona Adult Day Health Care Center (ADHC) located in Baldwin Park, Mr. Talib Abdullah, the social work field instructor, provided information about the center, such as their mission and the services ADHC offered to their participants. He also mentioned what was expected from the intern student. The first activity involved was to identify what kind of population the center served. It was rapidly identified that ADHC served participants from middle adulthood (approximately 40 to 65 years) to late adulthood (65 years and beyond). The majority of the participants served were Filipino. It was also detected that the second major groups were Hispanics and Latinos. Caucasian and Black participants were present, however, they were only a few. It was later required to be introduced as the new social worker intern and socialize with the participants. Feeling of anxiety arose when introducing personal. Communication was essential in assisting and getting to know the participants, however, there were a few language barriers with the Filipino group. Their primary language was Tagalog with limited English.

It is always great to learn something new. As a Nurse Educator at the James A. Haley VA Hospital, our focus is individualized care; Therefore, we use the HATCH Model. The PEN-3 Model is not a model that I remember hearing of, but I plan to do some more research about it. Using a model that focuses on individual's culture is important. I have witnessed many issues between health care workers and the patients, and those confrontations could have been avoided if healthcare workers would be more aware of culture differences and individualized care.
